The mission of Families In Transition is to prevent and break the cycle of homelessness with evidence-based approaches addressing the root causes of homelessness

We design programs and services using evidence-based models and practices to ensure that
people have the seamless support they need to find and maintain a home long-term. Our
goal is to partner with our clients and provide a continuum of care throughout their housing
journey.

Putting permanent housing first for families and individuals exiting homelessness creates a platform for them to achieve an improved overall quality of life. Finding employment, addressing health and wellness issues, and recovering from the traumatic impacts of homelessness are made possible with a stable home.

We offer emergency shelter for adults and families with children in two Manchester locations. Our emergency shelter programs provide safe and secure locations for families and individuals who are seeking refuge from homelessness.

Food Services

Proper nutrition is key in caring for the health and well-being of people experiencing homelessness and households at risk of becoming homeless.

Low-income households and those experiencing homelessness face significant challenges in the area of health and nutrition. Limited access to healthy foods can be a barrier to health. This often leads to a poor intake of vitamins and minerals and puts people at risk of malnutrition, which can often be overlooked when providing care

Substance Use Treatment

Addressing substance use disorders requires comprehensive and individualized care. Our Willows Treatment Center offers substance use treatment services, including outpatient and intensive outpatient services by licensed counselors and therapists to help people on their recovery journey.

We collaborate with community partners to ensure our clients have the support they need. We work with organizations in healthcare, first responders, and other local and state agencies and housing providers to deliver comprehensive solutions to our clients.
